When my husband David and I met about 12 years ago, it was actually pretty early in our relationship when we determined that both of us wanted to become dads one day. Even before we got married, we decided to actively pursue fatherhood together. We were excited when we got the news at about 6 weeks gestation that our surrogate was pregnant and we were having twins. We were also a bit terrified, mostly at the thought of having twins, and of the prospect of a high-risk pregnancy with the complications that could arise down the line. But everything progressed well during the coming weeks and months. The only significant problem was the size difference between the girls. One baby, Cecilia, was growing rapidly while Gabrielle was not growing as quickly. We were told that a threshold of a 25% difference between them would lead to complications; it was at 23% about 4 days before they were born. Twins often arrive early, but not always. We expected them to come around 36-37 weeks, which would have been early February, so we were in shock when we got a call from the doctors early in the morning in the 2nd week of January. The girls were born at 32 weeks after an incredibly short labor that required an emergency c-section. Gabrielle was born first at 3 lbs., 4 oz. and Cecilia came 2 minutes later at 4 lbs., 3 oz. To complicate matters, our surrogate lived in Mexico City. She received top quality prenatal care and the hospital was also high quality, but David and I were away from any family and our support system. It took about two weeks to coordinate things and get passports, but we were able to do a medical evacuation to El Camino Hospital. Cece spent 24 days total in three NICUs and Gabby spent 28 days in the NICU. Their biggest issues they faced were feeding and body weight. They were too small to maintain their body temperatures and taking anything more than just a few swallows of milk required too much energy. Gabby had jaundice and developed a sepsis infection in her blood. Luckily, they had received surfactant therapy, so their lungs were able to support their tiny brains and bodies and they didn’t require supplemental oxygen. Today, they are both healthy and strong. They are still on the small side for their age, but they are catching up. Developmentally they are on track. Gabrielle is still noticeably smaller, but she will always have the distinction of being the older sister.
